Diversity monitoring - A level student workshop June 24

Common Room wants to meet the aims and commitments set out in its Equality and Diversity Policy. This includes not discriminating under the Equality Act 2010, and building an accurate picture of the make-up of the the people we engage with through our work in encouraging equality and diversity.

We need your help to do this, by filling in this form. The information on this form will be used for monitoring purposes and shared with Contra Agency to give them an idea of the diversity of the participants of the workshops.

All questions are optional. You are not obliged to answer any of these questions but the more information you supply, the more effective our monitoring will be. All information supplied will be treated in the strictest confidence. It will remain anonymous and cannot be linked to you.
