Common Room North Ltd. wants to meet the aims and commitments set out in its Equality and Diversity Policy. This includes not discriminating under the Equality Act 2010, and building an accurate picture of the make-up of the workforce and the people we engage with through our work in encouraging equality and diversity.

We need your help and co-operation to enable us to do this, but filling in this form is voluntary. The information on this form will be used for monitoring purposes only to ensure we are meeting our Equality & Diversity Policy and will play no part in the recruitment process or your ability to engage with our work.

All questions are optional. You are not obliged to answer any of these questions but the more information you supply, the more effective our monitoring will be. All information supplied will be treated in the strictest confidence. It will remain anonymous and cannot be linked to you.
