Anonymous questionnaire: NHSE Inpatient CAMHS project

We need your help - we're really keen to build up an accurate picture of the different young people who take part in our project and some brief information about their experience of care.

You are not obliged to answer any of these questions and not completing all or part of it doesn't mean you can't take part, but the more information you supply, the better for us. Anything you tell us will be treated in the strictest confidence. It will remain anonymous and cannot be linked to you.
